 Providers:

We are all working hard to help get people with OUD access to treatment and need to know if your patients/clients are having trouble filling their prescriptions.

The below survey was developed by the New Mexico Department of Health to help them better understand any issues you are having with patients/clients being able to fill prescriptions for buprenorphine for OUD treatment (e.g., Suboxone, Subutex) at your local pharmacies. This survey only takes a couple of minutes to complete and will be extremely helpful to the state to understand the extent of the problem and to inform solutions. Thank you very much for your time and assistance!
